A $10 replacement fee is payable to replace all lost cards

To order a replacement card, log in to www.fitnesspassport.com.au with your username/email address and password.

Select “Manage Membership” on the left-hand side, and “Membership Cards”.

Select “Order New Card” next to the member needing a new card. Note: It is recommended that you upload a photo prior to ordering a new card, so if you need to upload a photo, you can do so here.

You will need to accept the $10 fee.

What happens next?

Your replacement card will be printed in the next card order and sent to your employer for distribution or posted directly to your registered mailing address within 2 weeks. Please ensure your address details are accurate in the Fitness Passport system in case your employer chooses to send the card to your home address (you can do this by clicking on the Contact Details tab and clicking on the ‘Edit Profile’ button if you need to update this).
